If you or someone you know is a caregiver for someone afflicted with Dementia, Alzheimer's Disease or some other form of cognitive impairment, you are strongly urged to register with Swansea's Silver Alert Program.

Special Needs Assistance Request - Emergency Help
Special Needs Assistance Request: Nothing is more important than the safety and well-being of all our Swansea residents. This is especially true during natural disasters and other emergencies that may leave us vulnerable. History has taught as that we need to be prepared for any emergency that may come our way. Can you imagine being trapped in your home during a blizzard with no electricity or heat? What would you do without power during a hurricane or in the days that follow? Now think about how these circumstances change if you're a senior citizen with physical or cognitive needs. We must recognizing the urgency of being prepared if we are to minimize the risks to our seniors. We are working with our emergency responders to do just that. In order to achieve that goal, we need to know how we can better serve you. A "Special Needs Assistance Request Form" is designed to open the lines of communcation now, in the event of an emergency in the future. Use this form to tell us about your special needs and who we can contact on your behalf in the event of an emergency.

Many Swansea seniors may be receiving assistance from Bristol Elder Services, Inc. (BESI). As you know, the Health Insurance Portability and Accountability Act (HIPAA) law prevents your information from being shared with anyone without your expressed written consent. Even though you may be receiving services through BESI, they are not at liberty to discuss your case or your needs with us without your permission. The Swansea Council on Aging is also bound by confidentially laws. Your information will not be shared, nor will it be made public. We are committed to working with you and with BESI to ensure you are receiving the best care possible.
